Forms - Water-Based Fire Protection Systems

Unless indicated otherwise, forms that require fees, signatures and/or other documentation must be mailed to the State Fire Marshal’s Office at 246 South 14th Street, Lincoln, NE 68508-1804.   This form must be completed and submitted to the State Fire Marshal’s Office for inspections conducted on water-based fire protection systems. The completed report must be mailed to the Lincoln office. Due to signature requirements this form cannot be emailed.

  Anyone wanting to be certified as a Water-Based Fire Protection Contractor must complete this application and mail it to the office with a check or money order for $100.
