Forms – Miscellaneous

Forms with a PDF format require Adobe Acrobat Reader to view the files. Forms with a format of DOC (Word), DOT (Word template) require Microsoft Office 2000 or higher to view. These files, except where the format is indicated as Online, must be saved to your hard drive prior to completing them. To download these files, right click on the link and select “Save Target as…” Specify the Desktop or another directory on your hard drive, such as My Documents, where you want to save the file. Unless indicated otherwise, forms that require fees, signatures and/or other documentation must be mailed to the State Fire Marshal’s Office at 246 South 14th Street, Lincoln, NE 68508-1804. If these forms are emailed it will delay processing.

    The local fire chief must complete this permit application and submit it to this office for approval. It is a three-page form. The first two pages are the same – one will be kept by this office with the affidavit and one will be returned to the fire chief after the application is approved or denied. The third page is an affidavit that requires the notarized signature of the property owner(s).

Fire Reports

    This form is to be used to request a copy of a fire or incident investigation report. Not all requests may be granted depending on the purpose for the request and the nature of the report. The agency legal counsel reviews all requests and determines whether they may be released. The form may be emailed to Carmen as an attachment.

Schools

  • Fire Drill Report

    All schools must complete this form to report the fire drills conducted during the school year. Fire drills should be conducted in accordance with Nebraska Fire Safety Codes. The DOT version may be emailed as an attachment to the appropriate District Office covering the area the school is located.
